Hémorragies intracrâniennes : épidémiologie et implication des traitements antithrombotiques

L’hémorragie intracrânienne (HIC) est une complication des traitements antithrombotiques. L’objectif de notre étude est d’évaluer la proportion d’HIC sous antithrombotique. L’objectif secondaire est d’appréhender une part « évitable » du fait d’un surdosage ou d’une indication non légitime. Nous avons réalisé une étude épidémiologique descriptive rétrospective monocentrique sur les HIC sur une période de 2 ans. Le type d’HIC, le type de traitement antithrombotique, le niveau d’anticoagulation et la légitimité de l’indication du traitement ont été analysés. Parmi les 400 patients hospitalisés pour HIC, 131 (33 %) étaient traités par antithrombotiques, anticoagulant anti-vitamine K (AVK) dans 14,1 % des cas et antiagrégants plaquettaires dans 15,1 %. Parmi les patients sous AVK, 30,2 % étaient en surdosage. L’indication du traitement était discutable dans 18,3 % des cas. Notre étude souligne la fréquence des HIC sous traitement antithrombotique, ainsi que l’importance de la part « évitable ». Intracranial hemorrhage (ICH) is an antithrombotic treatment complication. Our study’s goal is to assess the proportion of ICH occurring while the patient is on antithrombotic treatment. The secondary goal is to assess the proportion of “avoidable” ICH (anticoagulant overdosage, debatables indications). We conducted a descriptive epidemiological single-center study of ICH during 2 years. We analyzed the type of ICH, the type of antithrombotic treatment, the level of anticoagulation and the relevance of antithrombotic treatment indication. Of the 400 patients admitted for an ICH, 131 (33%) were traited by antithrombotic therapy: oral anti-vitamin K anticoagulants (VKA) in 14.1% of cases and antiplatelet agents in 15.1%. Of VKA patients, overdosage rate was 30.2%. The indication of antithrombotic therapy was debatable in 18.3% of cases. Our study highlights the frequency of ICH occuring on antithrombotic therapy and the significant proportion of “avoidable” ICH.
