本证为伏邪久羁脉络，痰瘀浊毒持续阻滞络中气血津液，日久由阻及损，络体受损、血脉失利所致。病至此阶段，瘀阻络损渐著而正气亦耗，故治当益气扶正、活血祛瘀、通络护脉，并兼顾受损络体，使瘀去络通而气血得行。方药宜兼具扶正、活血与通络之功。补阳还五汤由黄芪、当归尾、赤芍、川芎、桃仁、红花、地龙组成，重用黄芪益气扶正，配伍当归尾、赤芍、川芎、桃仁、红花活血祛瘀，地龙通络，共奏补气活血、祛瘀通络之功，与正虚血瘀、络脉痹阻之机较为契合
