Open peroral endoscopic myotomy for achalasia with failed Heller myotomy

Video S1 Preoperative endoscopy shows a tightly closed cardia. Submucosal injection and mucosal incision is created about 6 cm above the esophagogastric junction (EGJ) until it extends to at least 2 cm beyond the EGJ. We carried out this procedure on the posterior wall of the esophagus. Then, selective myotomy of the circular muscle is carried out. After complete myotomy, smooth passage of the endoscope through the EGJ with minimal resistance is possible.
