Facteurs associés à une mauvaise acceptation de la maladie chez les patients BPCO

Chez les patients atteints de bronchopneumopathie chronique obstructive (BPCO), l’acceptation de la pathologie est probablement un facteur majeur d’amélioration de la qualité de vie. L’objectif de ce travail est d’identifier les critères associés à une bonne ou une mauvaise acceptation de la pathologie et d’identifier les moyens de l’améliorer. Nous avons réalisé une enquête téléphonique au sein de l’ensemble des patients de l’association BPCO à l’aide d’un questionnaire standardisé établi au préalable par différents acteurs de santé. Sur les 1040 patients de l’association, 356 (34 %) ont répondu au questionnaire. 99 patients ont rapporté ne pas accepter leur pathologie (28 %). Les patients n’acceptant pas leur pathologie sont significativement plus sévères et ont plus de difficulté à réaliser les actes de la vie quotidienne, notamment une activité physique. Ces patients ont significativement plus de difficulté à comprendre leur pathologie mais rapportent aussi une attitude moralisatrice de leurs proches plus fréquente. Plus la pathologie est handicapante, plus l’acceptation est difficile pour les patients. Le médecin peut avoir un impact en améliorant l’éducation thérapeutique et en impliquant plus les proches dans la prise en charge. In patients suffering from chronic obstructive pulmonary disease (COPD), the acceptance of the illness is probably a major factor in the improvement of quality of life. The aim of this study is to identify the criteria associated with a good or bad acceptance of the disease and to identify means of improving it. We have undertaken a telephone enquiry among patients with COPD with the aid of a standardized questionnaire established by several health experts. Of the 1040 patients who have been contacted, 356 (34 %) replied to the questionnaire. Ninety-nine patients reported unacceptance of their disease (28 %). The patients who did not accept their disease were significantly more severe, with more difficulty in performing daily life activities, particularly exercising. These patients had significantly greater difficulty in understanding their disease and also reported more frequently a moralizing attitude among their family. The greater the handicap of the disease, the greater is the difficulty in accepting the disease by the patient. The doctor could have an impact in improving the therapeutic education and involving the family in the patient's care.
