(Cell 165, 35–44; March 24, 2016) We have updated the online version of our paper to include the accession numbers to the patient-derived whole exome datasets that were not available at the time of publication. The datasets can be accessed at the Sequence Read Archive (https://www.ncbi.nlm.nih.gov/sra) under the accession numbers SRA: SRP067938 (UCLA samples) and SRA: SRP090294 (UCLA and Vanderbilt samples).
