Myasthenia gravis can be generalized or localized as ocular myasthenia gravis. Case Report : We report an 5-year-old girl who presented with history of drooping of left eyelid since December 2020 and history of associated with diplopia and abnormal deviation of left eye . Examination revealed left side ptosis. A diagnosis of Juvenile Ocular Myasthenia gravis was made when symptoms improved with neostigmine challenge test. She was commenced on intramuscular Neostigmine challenge test which came as positive. Then she was treated with pyridostigmine and had a good response. Conclusion: Ocular Myasthenia gravis (OMG) is a rare disease in itself. A high index of suspicion is required in a juvenile as it is even rarer.
