Objective To observe the decentration of intraocular lens and the changes of the lens capsule, and to explore the protective measure of postoperative decentration. Methods Ninety-six eyes of 96 patients with age-related cataract were examined two months after phacoemulsification and IOL implantation. 96 eyes were divided into three groups according to the lens and the loops mode and compared with the decentration of the lens and the change of the capsules. Results The rates of the decentration ≥0.5 mm for three groups were respectively 50%、19%、15%,the rates of ≥0.5 mm were 21%、6%、3%. In eyes with the decentration ≥0.5 mm,the margin of the lens were completely covered with the anterior capsule in 17eyes(81%),the capsular capture happened in 5 eyes(24%). The lens moved to the widest anterior capsule and the vertical direction of the loops. Conclusion Capsular asymmetrical fibrosis is the main reason of the Decentration of intraocular lens,it is likely to occur in unfavourable loops of the lens and unsuccessful capculorhexis.
