摘要
We report the case of a 38-year old female patient diagnosed with ALK-positive non-small cell lung cancer (NSCLC) with brain metastases during pregnancy at the 24th week. The patient was treated with dexamethasone and low-dose radiotherapy until caesarian section at week 29. Following initiation of alectinib one day postpartum, the patient developed interstitial lung disease (pneumonitis) after six weeks and bilateral globus pallidus necrosis another three weeks later. Subsequent treatment with ceritinib had to be discontinued due to liver toxicity. Ultimately, ALK inhibitor treatment could be successfully switched to brigatinib with no pulmonary toxicity and recovery of neurological symptoms. Thus our case highlights potential complications of ALK-positive NSCLC in the postpartum period.
