The double-eyelid crease formation is primarily influenced by the attachment of the levator aponeurosis to the skin. The buried-suture method of double-eyelid blepharoplasty has gained popularity due to advantages like minimal scarring, faster recovery, and lack of suture removal. However, traditional techniques often rely on tactile feedback, leading to imprecise fixation and postoperative complications such as asymmetry and suture irritation. To address these limitations, this study evaluated the clinical effectiveness of a modified continuous figure-of-eight suturing technique between the subcutaneous and subconjunctival tissue for buried-suture double-eyelid blepharoplasty. A retrospective analysis was conducted on 28 female patients (mean age 24.9 years) undergoing this method from November 2022 to November 2023. Surgical designs were optimized with precise anatomic markings, and the suturing technique ensured consistent and natural double-eyelid creases. Postoperative evaluations at a 12-month follow-up demonstrated smooth, symmetrical, and natural double-eyelid shapes with minimal scarring. Complications such as ptosis, incomplete eyelid closure, or upper eyelid eversion were absent. Patient satisfaction was high, with 22 patients rating their results as satisfactory and 6 as relatively satisfactory. This innovative suturing method significantly enhances the precision and durability of buried-suture double-eyelid blepharoplasty. Its low complication rate, natural outcomes, and quick recovery establish it as a valuable advancement in cosmetic surgery, warranting broader clinical application.
