resistance to understand what is going on and there is no good animal model to study it'.Some 15 years ago, De Bruyne and Pijls applied the concept of bolus thermodilution to the coronary circulation to measure the mean transit time of blood through the heart, before advancing that to continuous thermodilution which enables measurement of absolute flow and absolute resistance in millilitres per minute, evolving to the microvascular resistance reserve (MRR) index to assess microcirculatory dysfunction, effectively CFR divided by FFR.'It is an elegant way of closing the loop of the study of the coronary circulation and it might well be the MRR might become for the microvasculature what FFR has been for the epicardial vessel' concluded Dr De Bruyne.'Now, we might wonder what Dr Gru ¨ntzig might have thought of these developments.Well, my hunch is that he would have loved it and even encouraged us to continue developing this precision medicine to further investigate the coronary circulation in the catheterization lab'.
