O bjectives:To determ ine and com pare the prevalence o f insulin resistance and carbohydrate metabolism param eters in wom en with endometrial pathology. Material and M ethods: 100 perim enopausal wom en with abnormal uterine bleeding and/or abnorm al endometriumw ere included into the study, H ystero sco p y with b iop sy w as perform ed, The study population w as divided into four groups according to histopathological results o f the endometrium: non-atypical endometrial hyperplasia, endometrial polyp, endometrial cancer, and controls, Fasting glu co se and insulin levels and OGTT, IR indexes, occu rren ce o f diabetes, pre-diabetic state, overweight, obesity, and hypertension were a s se sse d , R esu lts: Insulin resistance w as diagnosed in 41,0% o f the patients, The prevalence o f m arkers o f insulin resistance increased to 57,1% in c a s e s with confirm ed endometrial pathology, com pared to 31,8% in histologically normal endometrium (p<0,01), The frequency o f insulin resistance w as 52,6% (p=0,059) and 55 ,5 % (p=0,04), respectively, in wom en with non-atypical hyperplasia and patients with endometrial polyps when com pared to the control group, Abnorm al param eters o f carbohydrate metabolism indicate little sensitivity and specificity in predicting endometrial hyperplastic lesions, The insulin levels at 120 minutes o f O G T T correlate b e s t with su ch ch an ges (concentration> 57 pU/m l in c a se o f hyperplasia and >61 pU/m l in endometrial polyps), Conclusion: Insulin resistance and carbohydrate metabolism disturbances are com m on in w om en with endometrial pathologies, In th ese patients there is clinical ba sis for recom m ending lifestyle modification (change o f diet, more physical activity), o r for introduction o f pharm aceutical insulin-sensitizing agents,
