Background: Internal derangement of the temporomandibular joint (TMJ) is a significant contributor to orofacial pain and functional impairment. Effective treatment strategies are crucial to improving patient outcomes. Materials and Methods: A prospective comparative study involving 90 patients with confirmed TMJ internal derangement was conducted over 18 months. Patients were randomly divided into three groups, each receiving one of the therapies. Outcome measures included pain intensity, jaw function, and patient satisfaction, assessed at baseline, 6 weeks, and 3 months post-treatment. Results: All three therapies demonstrated improvements, with surgical interventions showing the highest pain reduction and jaw function improvement scores. Conservative management was effective in mild cases but less impactful for severe derangements. Minimally invasive therapies provided moderate relief, particularly for moderate cases. Conclusion: Tailored treatment approaches based on severity and patient-specific factors are essential for managing TMJ internal derangement effectively.
