Objective To detect the CD154 expression and NF-κB activity of peripheral blood mononuclear cell(PBMC) in patients with IgA nephropathy,and analyse the relationship between CD154 and renal tissue injury.Methods There 30 randomly selected patients with IgA nephropathy diagnosed by percutaneous renal biopsy were as observation group,another 30 randomly selected healthy persons were the control group.The expression of CD154 on PBMC was measured by flow cytometertheir and NF-κB activity in PBMC was measured by immunohistochemical method.The levels of serum creatinine,24-hr urine protein and urine NAG were determined and the results were anlayzed.Results The expression of CD154 on PBMC and NF-κB activity in PBMC in patients with IgA nephropathy were 15.1 ±4.7% and 33.8 ±10.5%,both were significantly higher than that in healthy persons(P0.01);(2)The cell number on the slit surface of glomerulus,the percent of renal tubular injury,the glomerular injury score,serum creatinine,24-hr urine protein and urine NAG were 87.7±30.3 个、 33.4 ±10.1% 、11.3 ±4.5% 、162.6 ±75.3 ummol/L 、2.215 ±1.361g/d、 62.3 ±35.4U/L.(3)CD154 expression on PBMC had a positive correlation with NF-κB activity in PBMC 、The cell number on the slit surface of glomerulus,、the percent of renal tubular injury 、the glomerular injury score 、serum creatinine 、24-hr urine protein and urine NAG(γ=0.475、γ= 0.396、γ= 0.538、γ=0.571(all P0.01) 、γ=0.308、γ=0.316、γ=0.324(P0.05)).Conclusion The expression of CD154 on PBMC in IgA nephropathy patients is significantly higher than that of healthy persons and the level is closely correlated with with NF-κB activity in PBMC,the parameters of renal tissue injury play a important role in the progress of IgA nephropathy that may be related to the interaction between CD154 and NF-κB.
