Analysis of Functional Outcome of Frozen Shoulder following Arthroscopic Capsular Release in Resistant Cases: Prospective study

INTRODUCTION: Shoulder stiffness is a manifestation of various pathologies or clinical scenarios. In the past, it has been variously described as scapula humeral periarthritis, frozen shoulder and adhesive capsulitis. It is a common but yet poorly understood disorder of the shoulder joint. Frozen shoulder is a clinical condition of uncertain etiology, which is characterised by significant restriction of active and passive motion of the shoulder that occurs in the absence of a known intrinsic disorder of the shoulder. The hallmark of this condition is a significant loss of both active and passive arc of shoulder movements. AIM OF THE STUDY: To analyse the functional outcome of frozen shoulder following arthroscopic capsular release in resistant cases. MATERIALS AND METHODS: Study Design: Prospective study. Study Period: One year. Sample size: 20 cases were taken up for our study. The study was conducted in 20 patients with frozen shoulder. Arthroscopic assisted controlled capsular release was done in all patients after clinically examining each patient. All patients were evaluated post operatively at regular intervals for a period of 1 year using ASES scoring system. Inclusion Criteria: • Diabetics (controlled), • Post traumatic related stiffness, • Post surgical related stiffness, • Osteoporotic individuals, • No response to conservative treatment (NSAIDS, ROM exercise), • Inability to get full ROM even after gentle MUA. Exclusion Criteria: • -Rotator cuff injury, • Significant labral injury, • Injury to the long head of biceps, • Intra articular fractures, • Arthritis. RESULTS: The statistical data were analysed using mann whitney U test. All patients had showed significant improvement in post operative painless range of movements. There was no difference in the muscle power postoperatively including abduction, external rotation and internal rotation. CONCLUSION: Resistant cases of the frozen shoulder can be effectively managed by arthroscopic controlled capsular release to obtain painless range of movements and to avoid complications
