Loose anagen hair

An 11-year-old girl presented with diffuse hair loss and slow-growing hair for 3 years. Her hair could be easily and painlessly extracted. Light microscopy revealed plucked anagen hair demonstrated misshapen hair roots. Under scanning microscopy, distorted shaft immediately distal to the bulb was absent of cuticle, and the ruffled cuticle surrounded the proximal hair shaft.
