In patients with coronary artery disease, residual cardiovascular risk remains substantial even after levels of low-density lipoprotein (LDL) cholesterol have been effectively lowered by treatment with statins, ezetimibe, or antibodies targeting proprotein convertase subtilisin–kexin type 9 (PCSK9). Many of these patients have elevated levels of triglyceride-rich lipoproteins, often in the context of obesity and insulin resistance.
