The pathogenesis of diabetic nephropathy(DN) is intricate and complex.Polyol pathway, protein kinase C,advanced glycation end products, and oxidative stress play an important role in the progression of DN.These mechanisms are not independent as they are inter-related and interacted in the pathogenesis of DN caused by chronic hyperglycemia. The activation of polyol pathway may be the initiating and promoting factor, whilst aldose reductase plays a pivotal role as a key regulatory.
