Abstract P-140: COMPARATIVE STUDY OF ACETATED ISOTONIC ELECTROLYTE SOLUTION, NORMAL SALINE SOLUTION, AND LACTATED RINGER’S SOLUTION IN THE INITIAL FLUID RESUSCITATION OF CHILDREN DIAGNOSED WITH SEVERE DENGUE
Aims & Objectives: Local studies are lacking comparing the use of acetated isotonic electrolyte solution against normal saline and lactated Ringer’s solutions in the initial fluid resuscitation among severe dengue patients. This study aims to determine the comparative recovery time to achieve initial and sustained cardiovascular stability among severe dengue patients. Methods Retrospective cohort study of severe dengue patients aged 1 month to 18 years old admitted at the intensive care unit from 2014 to 2016. They were divided into 3 groups based on the initial fluid used. Demographic data, course, and outcomes were reviewed. Results There were 166 patients included in this study. AIES had the shortest time to achieve initial stability (2.29 vs 2.5 vs 2.7 hours; p = 0.03) and sustained stability (4.4 vs 4.6 vs 4.9 hours; p = 0.01). AIES had less hypernatremia (p < 0.05) and hyperchloremia (p <0.05) as evident by normal anion gap and decreased strong ion difference. Although not statistically significant, there was a trend that AIES had less re-shock, less colloid and inotropes used, less blood transfusion, and less use of mechanical ventilation and need for dialysis (p > 0.05). Length of ICU stay was almost the same in all groups and not statistically significant. No mortality was recorded on the acetated isotonic electrolyte solution group as compared to 3 in normal saline solution and 2 in lactated Ringer’s solution groups. Conclusions Acetated isotonic electrolyte solution is more effective than normal saline and lactated Ringer’s solutions in the initial fluid resuscitation among patients with severe dengue.
