The study on therapeutic efficacy and reliability of the endovascular embolization in brain arteriovenous malformation

Objective To evaluate the therapeutic efficacy and reliability of the endovascular embolization in brain arteriovenous malformation.Methods We retrospectively analyzed one hundred and six cases of BAVM from Sep,2004 to Sep,2009 who were followed up for 3-36 months with an average follow up of 26.37 months.Patients were divided into two groups:the assistant emolization group in which 28 patients underwent the NBCA embolization in 28 cases and the curative embolization group in which 72 patients received the Onyx embolization.Results In the assistant emolization group,102 arteries were embolized in 65 surgical operations and the average successful embolization rate was 42.8%.The 100% and the 80-99% embolization were achieved in 4(14.3%)and 3(10.7%)cases,respectively.The operation-related complication was 4.6% including one disabled case(3.6%).During the followed up,the 100% embolization rate remained 7.1%.In curative embolization group,194 arteries were embolized in 137 operations and the average sucessful embolization rate was 76.4%.The 100% and the 80-99% embolization was obtained in 30(38.5%)and 25(32.1%)cases,respectively.The operation-related complication was 13.9% including 3 disabled cases(3.8%).The 100% embolization rate remained 28.2% during the followed up.Conclusions Curative endovascular embolization can achieve successful embolization in almost 1/3 patients with BAVM.Although the curative embolization has a higher morbidity of the operation-related complication,it's morbidity of the neurological deficit and disable rate is similar to that of assistant embolization.Both the curative embolization and assistant embolization do not cause any mortality.Therefore,the curative embolization is reliable and feasible in treating BAVM.
