Digital integration has also extended to orthognathic surgery, allowing clinicians to overcome previous limitations related to periodontal architecture. This case report illustrates a fully digital approach to preparing for a dentoskeletal class III malocclusion for bimaxillary surgery. The patients underwent orthodontic treatment using Invisalign Clear Aligners to reverse the dental compensation associated with this malocclusion and enable the surgical repositioning of the maxillary and mandibular bones through maxillofacial surgery. Moreover, to ensure safe dental movement, periodontal support was provided through ODO (Orthodontically Driven Osteogenesis), a surgical technique combining selective corticotomy, particulate bone grafting, and orthodontic movement. “The patient underwent a Le Fort I osteotomy of the maxilla and a bilateral sagittal split osteotomy of the mandible. Fixation was achieved using custom-made titanium plates and screws to ensure stability. A refinement stage with aligners followed to accomplish a bilateral Class I molar and canine relationship(alignment?).”
