C4 root compression leads to phrenic nerve palsy with shortness of breath, chest pain, and elevated hemidiaphragm

A 50-year-old man presented to our emergency department complaining of sudden onset of shortness of breath and pain in the back of his right shoulder. Notably, he did not report any neck pain, numbness, or weakness in his arms or legs; he had no abdominal pain or fever.
