Objective To observe the clinical efficacy and safety of intravenous thrombolytic therapy using reteplase(r-PA) or urokinase in acute myocardial infarction(AMI).Methods Ninety-eight patients with AMI during March 2006 to October 2007 were randomized to receive either reteplase or urokinase.Reteplase was administered as 2 intravenous bolus injections of 10 MU 30 minutes apart;Urokinase was administered as an intravenous infusion of 1.5×106u in 30 min.The infarct-related coronary artery recanalization rate,fatality rate in acute stage,incidence of complications and adverse reactions of the two groups were compared.Results The recanalization rate by clinical judgment at 120 min was higher in reteplase group than in urokinase group,with statistically significant difference at the two groups(P0.05);The recanalization rate by angiography was also higher in reteplase group than in urokinase group(85.42%vs72%).The fatality rate in 35 days was 4.16% in reteplase group and 10% in urokinase group.Conclusion Reteplase is an effective,reliable and safe thrombolytic agent in the treatment of acute myocardial infarction.
