Label-free approaches minimize the inherent low reproducibility of SERS substrates by lowering fabrication steps of the final nanosensor. Further, using label-free strategies it is possible to have direct information about the biological entities or events under scrutiny. In this chapter, we are presenting some examples that are paving the way toward the translation of label-free SERS technologies as POC testing in different biomedical applications. Label-free SERS may fill the gap in the POC field bringing a higher selectivity and potential multiplexing, without compromising selectivity, miniaturization, automation, and/or connectivity. Specifically, we herein explore the state of the art of SERS as potential POC testing in the clinic for cancer, neurological and infectious diseases.
