The aim of our study was to evaluate the accuracy of trans-vaginal ultrasound combined with colour Doppler for the discrimination of benign and malignant purely solid ovarian tumours. Sixty-six consecutive women with pure solid ovarian tumours were evaluated retrospectively over a period of 12 years. Sonographic parameters were tumour size, regularity, intensity of the acoustic shadow (low, medium or high referring to peripheral tumour margins), presence of ascites, tumour vascularity (absent, low or high) and mobility. Final histopathological diagnosis was correlated with pre-operative sonographic tumour characteristics. Histopathological examination revealed 50/66 (75.5%) benign tumours and 16/66 (24.5%) malignant tumours. No statistical difference was found in regard to final pathological diagnosis for demographic characteristics (age, parity, and menopause) as well as for tumour size, regularity of the tumour walls, presence of ascites and mobility. High intensity acoustic shadow was evident solely in benign tumours (p < 0.05). Rich vascularity was statistically significantly correlated with malignant histology (p < 0.001). The combination of low or intermediate acoustic shadow and rich vascularity revealed an odds ratio of 108 for malignancy.
