Inflammatory bowel diseases (IBD) have experienced a concerning global increase in incidence over recent decades. The advent of molecular imaging probes has revolutionized real‐time biomarker monitoring in IBD pathophysiology, enabling visualization of critical pathological processes. These probes provide dynamic insights into oxidative stress mediators (including reactive oxygen species and reactive nitrogen species) and enzymatic biomarkers (e.g., caspase‐1, pantetheinase, dipeptidyl peptidase IV, nitroreductase, elastase, myeloperoxidase, and carboxylesterase 2), which are crucial for understanding disease mechanisms. Despite significant technological advancements, there remains a lack of systematic analyses exploring the diagnostic potential and therapeutic monitoring applications of these molecular probes in IBD management. This review comprehensively analyzes the design of state‐of‐the‐art molecular probes targeting IBD‐specific biomarkers, with particular focus on their detection mechanisms and biological applicability. Through a mechanistic elucidation of detection paradigms, this review aims to guide the development of future probes for diagnosing IBD.
