Effects of larger dosage of spironolactone on the heart rate variability and ventricular remodeling in elderly patients with chronic heart failure

Objective To observe the changes of heart rate variability(HRV),ventricular remodeling parameters after intervention of larger dosage of spironolactone.Methods Totally 120 patients with chronic heart failure(CHF) were randomly divided into conventional therapy group,smaller dosage of spironolactone group(20 mg/d) and larger dosage of spironolactone group(60 mg/d).HRV and cardiac function parameters were measured before and 3 months after intervention.Results After 3 months of therapy,HRV and LVEF were all improved in smaller dosage of spironolactone group and larger dosage of spironolactone group.LVEDD,LVESD were reduced and LVEF was increased in two spironolactone groups(P0.05),and more obviously in larger dosage of spironolactone.Conclusions The larger dosage of spironolactone can obviously improve HRV and ventricular remodeling in elderly patients with CHF.
