Osimertinib with Chemotherapy in EGFR -Mutated NSCLC

To the Editor: In the report by Planchard et al. (Nov. 23 issue) 1 on the FLAURA2 trial of osimertinib with or without chemotherapy in EGFR -mutated advanced non–small-cell lung cancer (NSCLC), several topics were missing in the discussion, and we seek the authors’ opinion. First, we doubt that up-front combination therapy is better than sequential treatment, given that overall survival seemed similar in the osimertinib–chemotherapy group and the osimertinib group, that there was a lack of evidence for synergism (with a similar depth of response in the two groups), and that subsequent cytotoxic chemotherapy in the monotherapy group was not mandatory.
