Elevated plasma LDL cholesterol is a major cause of atherosclerotic cardiovascular disease. Accordingly, all clinical practice guidelines advise decreasing LDL cholesterol to very low concentrations (<1·4 mmol/L),1,2 particularly in high-risk patients with established atherosclerotic cardiovascular disease.1 Statins are a backbone of treatment, but tolerability to these drugs can be an issue and co-administration with other oral antilipaemic drugs, typically ezetimibe, might have little additional benefit in lowering LDL cholesterol.
