Abstract Mechanisms underlying reflex seizures are varied and diverse, potentially stemming from various etiologies. EEG studies emerge as an indispensable tool for accurate diagnostic framing. Analyzing background and interictal activity can provide crucial data in this regard, but the primary objective of EEG studies is recording the reflex event. This is essential to confirm the epileptic nature of the manifestation, which can be subtle—or, conversely, to ascertain the non-epileptic nature of seizure-mimicking events. In addition to standard stimulation tests, a meticulous patient history can guide the execution of tailored tests during EEG recording aimed at identifying the exact trigger, which is often elusive or challenging to identify. This has significant implications for defining outcomes and implementing appropriate treatment strategies. This chapter provides several examples and some suggestions for conducting appropriate EEG recordings for these purposes.
