We report the case of a 27-year-old female with recurrent eosinophilic pneumonia treated successfully with mepolizumab, an anti-interleukin-5 monoclonal antibody that targets eosinophilic inflammation. The patient had a several year history of recurrent eosinophilic pneumonia with declining lung function, and was steroid dependent, with disease refractory to azathioprine and mycophenolate mofetil. Regular mepolizumab infusions have afforded complete remission from eosinophilic pneumonia without the use of corticosteroids.
