摘要
Objective To observe the effect of combined treatment with synchronized intermittent mandatory ventilation(SIMV)and pressure support(PS)on chronic obstructive pulmonary disease(COPD)and its clinical significance.Methods Ninety-two patients with COPD were given anti-infective therapy,anti-primary disease therapy,nutritional therapy and other conventional therapies.In addition,patients were randomly treated with SIMV alone(control group,n=45)or in combination with PS(observation group,n=47).Changes in respiratory frequency,tidal volume,minute ventilation,peak inspiratory pressure(PIP),partial pressure of carbon dioxide(PaCO2),partial pressure of oxygen(PaO2),oxygen saturation(SaO2)and pH value were observed in both groups.Furthermore,mechanical ventilation time,time to liberation from mechanical ventilation,rate of successful liberation from mechanical ventilation and length of ICU stay were recorded in all patients.Results There were no significant differences in respiratory frequency,tidal volume,minute ventilation,PIP,PaCO2,PaO2,oxygen saturation SaO2 and pH value between the two groups(P0.05).Mechanical ventilation time,length of ICU stay,time to liberation from mechanical ventilation and rate of successful liberation from mechanical ventilation were,respectively,(13±9)days,(15±10)days,(60±5)hours and 92.0%in observation group,and(21±11)days,(22±8)days,(73±8)hours and 73.8%in control group(P0.05).The differences were obvious between the two groups(P0.05).Conclusion The combined treatment with SIMV and PS can shorten the time to liberation from mechanical ventilation,reduce ventilator dependency and promote recovery in patients with COPD.
