Quelles stratégies thérapeutiques pour les CBNPC de stade IIIB (T4N2/T1-T3N3) ?

Environ un quart des cancers bronchiques non à petite cellules (CBNPC) présentent au diagnostic un stade localement avancé (III). Ce stade regroupe des maladies hétérogènes en terme de présentation clinique initiale (tumeurs localement avancées T4 et/ou atteinte ganglionnaire médiastinale controlatérale N3) et d’histologie. Un staging initial optimal intégrant une imagerie TEP et une imagerie cérébrale injectée est nécessaire. Des EFR sont à réaliser systématiquement dès lors qu’un geste chirurgical est envisageable mais également préalablement à la réalisation de la radiothérapie. La stratégie thérapeutique se décide en réunion de concertation pluridisciplinaire au sein d’une équipe expérimentée. La chirurgie est une option envisageable dans certains stades T4 et est en général précédée d’un traitement néo adjuvant. La majorité des CBNPC de stade IIIB et IIIC relève d’une indication de radiochimiothérapie, un schéma concomitant est à privilégier à un schéma séquentiel si l’état général du patient le permet.
