Recent work in psychedelic science reports durable reductions in fear of death following psychedelic-assisted interventions and naturalistic psychedelic experiences. A prominent interpretation, developed by Letheby, holds that this effect is driven mainly by the adoption of non-physicalist metaphysical beliefs, especially beliefs in postmortem survival. I argue that current evidence does not justify treating such belief change as the universally primary mediator of psychedelic-related reductions in death anxiety. I propose the Confidence Reorientation Account (CRA), which locates the more general therapeutic pathway in a structural reorientation of the self-model: a shift in the hierarchical confidence, salience, and threat-weighting of self-related priors. Decomposing death-related threat into target, meaning, and weight, I argue that fear reduction can occur through a reconfiguration of threat representation, expressible within a predictive-processing framework in which psychedelics relax the precision of high-level self-related priors. Metaphysical belief change may, for some individuals, mediate, accompany, or consolidate this reorientation; for others, fear reduction may occur with little durable belief change. CRA is therefore best understood as a broader, multilevel framework that accommodates a causal role for belief change while generating testable predictions about mediation, temporal order, and moderation.
