Impact of Ethical Climate and Ethical Leadership Perceived by Nurses on Moral Distress

본 연구는 도덕적 고뇌, 윤리적 풍토, 윤리적 리더십의 정도를 확인하고 간호사가 지각한 윤리적 풍토와 윤리적 리더십이 도덕적 고뇌에 미치는 영향요인을 검증하기 위해 시행된 서술적 조사연구이다. 연구대상은 서울 및 경기지역 종합병원 및 대학병원 4곳에 근무하는 간호사 222명을 편의추출 하였고 자료수집 기간은 2016년 7월부터 9월까지였다. 수집된 자료는 SPSS/WIN 24.0 프로그램으로 기술통계, t-검정, 일원분산분석, Pearson 상관계수, 다중회귀분석을 이용하여 분석하였다. 연구결과, 윤리적 풍토(r=-0.267, p<0.001)와 윤리적 리더십(r=-0.227, p=0.001)이 모두 도덕적 고뇌와 통계적으로 유의한 음의 상관관계를 보였다. 도덕적 고뇌에 영향을 미치는 요인은 윤리적 풍토의 하부 영역 중 관리자와 관련된 윤리적 풍토(ß=-0.318, p=0.001), 윤리적 리더십의 하부 영역 중 관리자의 윤리지침(ß=-0.271, p=0.001), 인간중심(ß=0.255, p=0.001)의 순으로 나타났다. 근무부서 중 응급실(ß=0.575, p<0.001)과 소아과 또는 부인과 병동(ß=0.122, p=0.030)에 근무하는 간호사가 일반병동의 간호사보다 도덕적 고뇌가 통계적으로 유의하게 높은 것으로 나타났으며 이들 변인의 설명력은 47.9%이었다(F=41.584, p<0.001). 본 연구결과를 토대로 간호사의 도덕적 고뇌를 완화하기 위해서는 조직은 긍정적인 윤리적 풍토 조성과 관리자의 윤리적 리더십 증진에 관심을 가져야 하고, 근무부서의 윤리적 맥락의 특수성을 고려할 필요가 있다.The purpose of this study was to investigate the relationship among ethical climate, ethical leadership and moral distress and the impact of ethical climate and ethical leadership on moral distress among nurses. A descriptive and correlational research was adopted by a convenience sample of 222 nurses from 4 general hospitals in South Korea. Data was collected between July and September in 2016. Data analysis was performed by SPSS/WIN 24.0 program for t-test, ANOVA, Pearson correlation coefficient, and multiple regression analysis. As a result, moral distress is higher for nurses who had a less ethical perception of ethical climate and their managers` leadership. Furthermore, the ethical climate related to managers and the ethical leadership related to ethical guidance, and people orientation perceived by nurses were significantly influential factors on moral distress. The nurses in emergency room and pediatric and gynecology unit experienced significantly higher levels of moral distress than the nurses in general unit. In conclusion, nursing leaders should demonstrate ethical attitudes and behaviors based on clear ethical standards and healthcare organizations should take responsibility for creating high ethical working environment as well in order to lessen moral distress among nurses.
