Transpedicular fixation combined with vertebral plasty in treatment of senile thoracolumber burst fracture

Objective:To study the feasibility of transpedicular fixation combined with vertebral plasty in the treatment of senile thoracolumbar burst fracture and evaluate the result.Method:23 patients who were more than 60 years old with thoracolumbar burst fracture were treated with transpedicular fixation combined with vertebral plasty.The change of compressed ratios between anterior and posterior edge of vertebral body postoperatively,the improved Cobb angle were observed respectively.All cases were followed up.Result:The mean value of anterior compressed ratios was (52.7±5.07)% preoperatively and (8.09±0.83)% postoperatively.The mean value of posterior compressed ratios was (18.12±2.29)% preoperatively and (0.99±0.22)% postoperatively.Furthermore Cobb angle was improved.All cases were followed up 12~40 monthes.10 cases that spinal cord and nerve that injured were reached grad Ⅰ by ASIA.Conclusion:Transpedicular fixation combined with vertebral plasty is an alternative method in treatment of senile thoracolumbar burst fracture,it can restore the height and strength of the vertebral body,and rebuild the stability and reduce the complications such as the loss of height,nail breakdown etc.
