Objective: To explore the security of improved laparoscopic hysterectomy for large uterus.Methods: Two hundred and sixty-seven patients who underwent laparoscopic hysterectomy due to gynecological benign lesions from Jan.2009 to Jul.2012,were divided into control group and study group.The 189 cases in control group(uterus ranged from withered to pregnancy 11 weeks) used conventional method;78 cases in study group(12-22 weeks pregnant uterus) used a modified method;Then,the intraoperative situation,the postoperative recovery,the hospital stay,postoperative complications and follow-up situation were compared between the two groups of patients.Results: Two cases were converted to laparotomy in the study group and the conversion rate was significantly less than the control group(P 0.05),but intraoperative blood loss,postoperative recovery,hospital stay,complications were not significantly different(P 0.05).Conclusions: Improved laparoscopic hysterectomy for big uterus is secure and feasible,as long as laparoscopic surgeons are skilled,indications are reasonable,perioperative management is strict,this procedure will not increase postoperative morbidity,and is suitable for clinical application.
