PREDICTING THE RISK OF CARDIOVASCULAR EVENTS WITH AMBULATORY BLOOD PRESSURE MONITORING IN ELDERLY HYPERTENSION

Objective To evaluate the risk of cardiovascular events with ambulatory blood pressure monitoring(ABPM)in elderly hypertension. Methods 68 cases of elderly hypertension were measured with ABPM. Blood pressure readings were obtained automatically at 30-minute intervals at daytime and at 60-minute intervals at night time. All the parameters of ABPM were analyzed and compared with elderly normal group. The correlativity between the risk of cardiovascular events and parameters of ambulatory blood pressure were analyzed. Results The 24-hours SBP and day time SBP and night time SBP and blood pressure load in the elderly hypertensive group were significantly higher than the elderly normal group, but the 24-hours DBP especial night time DBP were not higher. 24 hours systolic pressure loading are 43.2±16.9 as compared with normal elderly group 15.6±14.5, p0.05. The 24-hours mean pulse pressure (mpp) in the elderly hypertensive group are 68.8±9.3 as compared to the normal groups are 42.2±9.5, t=2.08, p0.05. When the mpp was larger than 60 mmHg in elderly hypertensive group, the occurrence rate of the coronary heart disease was 58.8%(23 cases),the cardiovascular events 43.6%(17 cases),but only 31% (9 cases) and 20.6%(6 cases)when the mpp was lower than 60 mmHg.It is significantly in statistic 2=1.98 p0.05. Conclusion When the parameters of mpp, non-dipper, extreme-dipper blood pressure and blood pressure loading increase in the elderly hypertension, the risk of cardiovascular events increase. Measures must be taken to regulate these parameters in order to reduce the risk of cardiovascular events.
