This chapter provides the basic characteristics, side effects/toxicity, drug interactions, and dosage information of the Biaxin (Clarithromycin). Clarithromycin acts by binding to the 50S ribosomal subunit of susceptible microorganisms and, thus, interfering with microbial protein synthesis. Clarithromycin can be administered with or without food. Clarithromycin and other macrolides are known to inhibit enzymes, particularly CYP3A; therefore, coadministration of clarithromycin and a drug primarily metabolized by CYP3A may be associated with elevations in concentrations of the latter. Biaxin is administered in 250 mg and 500 mg tablets; and as granules in 2 concentrations: 125 mg/5 mL and 250 mg/5 mL. Biaxin XL is administered in 500 mg tablets. The recommended dose of clarithromycin for the prevention of disseminated Mycobacterium avium disease is 500 mg bid. When treating MAC, clarithromycin must be combined with other agents to minimize resistance. Clarithromycin should not be used in pregnant women unless there is no alternative.
