Characteristics and choice of the operation of compound acetabular fracture via a posterior approach

Objective To explore the characteristics and choice indications on treating compound acetabular fracture via a posterior approach(Kocher-Langenbeck).Methods From Oct.1994 to Jan.2010,59 cases of compound acetabular fracture were treated by posterior approach,and the operation procedure,postoperative restoration conditions and complications were evaluated.Results For 59 cases,the average operation time was 105 minutes,average amount of bleeding was 480ml,and average blood transfusion was 2U.Thirty-one cases achieved anatomical reduction,17 cases satisfactory reduction,11 cases unsatisfactory reduction;heterotopic ossification occurred in 11 cases.Conclusion To the compound acetabular fracture,posterior approach would have better effect on the fractures of posterior wall and posterior column,transverse and posterior wall fractures,T type and anterior column and posterior transverse fracture,or some double-columns fracture.This surgical approach has advantages of less surgery-related trauma,good reduction and lower incidences of complications.
