GOLD stage 1 COPD pharmacotherapy: big questions, few answers

Despite GOLD 1 accounting for roughly half of the 200–400 million people with COPD worldwide, randomised controlled trials supporting the routine use of pharmacotherapy in this group are lacking. Better, more considered studies are sorely needed.
