“Amniotic fluid sludge” is the presence of particulate matter in the amniotic fluid in close proximity to the cervix. Patients with “amniotic fluid sludge” have a significantly high frequency of MIAC, intra-amniotic inflammation, clinical and histologic chorioamnionitis. However, “sludge” is also present in about 4% of pregnancies during transvaginal ultrasound in the first and early second trimestre. We report a clinically homogeneus cohort of four cases who presented amniotic fluid sludge during transvaginal ultrasound linked to cervical inssuficiency and amniotic sac prolapsed between 20 and 24 weeks. None of our cases accomplished maternal Gibbs chorioamnionitis criteria on admission. Lower genital cultures resulted positive for Ureaplasma urealyticum in all our cases. Intraamniotic infection was confirm by amniocentesis since amniotic fluid samples which were removed from the “sludge” showed low glucose levels and high graim stain. Besides Fusinobacterium nucleatum was isolated in two samples. Fetal expulsion ocurred within the first 24-48 hours of diagnosis and the pahological diagnosis of our placenta showed chorioamnionitis in all of them and the second case also presented funisitis.
