Case Report: A 41-year-old female was admitted into the hospital because of right lower quadrant pain for the last two with edema in the lower right limb.The patient's past medical history includes tobacco, no oral contraceptive pills and no surgeries.She had been pregnant three times with no problems in the pregnancy period or in the delivery.Her mother died because of a stroke and her brother had a deep vein thrombosis episode.On physical examination, vital signs were normal, in her abdominal examination was remarkable for pain in the lower quadrant with rebound tenderness and peritoneal signs.Laboratory results revealed no abnormalities.Chest X-rays and electrocardiogram showed nothing remarkable.She went into an abdominal surgery that revealed a normal appendix.Because she continued with pain, a computed tomography (CT) of the abdomen and pelvis was ordered.CT findings were compatible with an acute ovarian vein thrombosis and right hydronephrosis.The patient was placed on anticoagulant theraphy with no progression of the thrombosis to the inferior cava vein and no pulmonary embolism.Laboratory test done to investigate coagulation disorders, bacterial infection or cancer revealed nothing abnormal.Repeat CT performed 3 later disclose no residual thrombosis.Conclusion: This case illustrates the difficulty of the clinical diagnosis of ovarian vein thrombosis and the importance of its inclusion in the differential diagnosis of female acute abdomen.Patients with OVT occasionally undergo laparotomy for presumed appendicitis before the correct diagnosis is established.Ovarian vein thrombosis must be recognized and treated early to reduce the risk of serious complications.
