Current 1L standard of care treatment for patients (pts) with ES-SCLC, chemo-immunotherapy (chemo-IO; platinum-etoposide with anti-PD-L1, followed by maintenance with an anti-PD-L1), demonstrates a median overall survival (OS) of 12–13 months (mo). However, most pts relapse within 6 mo, highlighting the need for more effective therapies. In the phase III DeLLphi-304 study, tarlatamab as second-line monotherapy significantly improved OS and progression-free survival (PFS), reducing the risk of death by 40% vs chemotherapy (NEJM 2025;393:349-361).
