Research on the formation of logistics information and establishment of fuzzy evaluation model

Based on supply chain, the paper analyzes the formation of logistics information, and concludes the layer and covering modules of it. Then, according to the construction of logistics enterprise information we establish the objective and efficient four-dimension logistics enterprise information evaluation index system. What's more, using triangular fuzzy number, we apply the evaluation model to appointed enterprise.
