The introduction of intravenous thrombolysis for acute ischaemic stroke has changed the outlook of stroke neurology, altering the field from a rehabilitation focus to emergency medicine. Because the treatment effect of intravenous thrombolysis in acute stroke is time dependent, stroke services globally are constantly striving to reduce the time from symptom onset to treatment by promoting stroke awareness campaigns, implementing prehospital delivery of thrombolysis in mobile stroke units, and restructuring in-hospital protocols to reduce door-to-needle time.
