Whether physical activity and/or chronotype may reduce cognitive frailty-a relatively new “compound” phenotype proposed in 2013-and whether the synergistic effect between physical activity chronotype and the risk of cognitive frailty are unknown. Therefore, this study aimed to investigate the association between daily physical activity, chronotype and risk of cognitive frailty in a Korean older population. Subject were 232 community-dwelling healthy individuals aged 70–85 years. Total volume of activity at a moderate to vigorous intensity and timing of activity and sleep pattern were measured by 3 axial accelerometer throughout each 24-h period for 1 month. Participants also completed the morningness-eveningness questionnaire, which is a 19-item self-report scale with questions focusing on habitual waking and bed times. The association between daily activity, chronotype and risk of cognitive frailty was determined using a multiple logistic regression model. Age- and sex-adjusted incidence rate of cognitive frailty was significantly greater in subjects with eveningness chronotype and low active than morningness and high active. These associations remained unchanged after adjustment for potential confounding factors (eveningness: odds ratio (OR)=2.32, 95% confidence interval (CI)=1.21–4.13 for cognitive frailty; Low active (<12min/day of MVPA): OR=2.69, 95% CI=1.27–4.88 for cognitive frailty). In this cross-sectional study design, causation cannot be inferred. Nevertheless, we suggest that from the viewpoint of cognitive and physical health, older population should be encouraged to engage in low- and moderate-intensity habitual physical activity, taking a duration > 12 min/day at a moderate intensity. Moreover, these results suggest that elderly with higher eveningness chronotype also have higher risks of cognitive frailty.
