The Relationship Between The Nuclear Dna Content In Smears Of Aspirates And The Prognosis Of Mucoepidermoid Carcinoma

Cytophotometric analysis of nuclear DNA content was performed in a series of mucoepidermoid carcinomas with an extreme difference in the clinical course, which was observed during a follow-up period of 6–13 years. The prerequisite for such a study was the development of a method, which made it possible to study the nuclear DNA content in tumour cells obtained at the time of diagnosis, i.e. 6–13 years ago in the present material. A special cytochemical procedure with destaining of the original Giemsa stain, refixation and subsequent Feul-gen staining of the smears of aspirates in the original cytological material was developed. The cytophotometric nuclear DNA analysis of tumour cells in smears of aspirate from mucoepidermoid carcinomas showed that higher ploidy tumours (near-triploid) had a worse prognosis when compared with near-diploid tumours. These data are supported by our previous findings that a shift from a near-diploid to a near-triploid DNA content of the tumour cell nuclei was associated with the prognostic important property of invasive growth. Thus, the cytophotometric nuclear DNA analysis of aspirated tumour cells seems to give valuable information about the malignancy degree of the individual tumour, which is not possible to obtain only from the cytomorphological features.ZUSAMMENFASSUNGAnhand einer Serie von Mukoepidermoid-Karzinomen mit extrem unterschiedlichem klinischen Verlauf wurde eine cytophotometrische Analyse des nuklearen DNS-Gehalts durchgeführt. Diese Gruppe war während eines Zeitraums von 6 bis 13 Jahren in fortlaufender Observation. Vorbedingung für diese Analyse war die Ent-wicklung einer Methode, die es ermöglicht, den nuklearen DNS-Gehalt in Tumorzellen, die zur Zeit der Diagnose, d. h. in dem vorliegenden Material von 6 bis 13 Jahren, entnommen worden waren, zu untersuchen. Es wurde daher eine spezielle zytochemische Prozedur entwickelt, die Entfärbung der ursprünglichen Giemsa-Färbung, Neufixierung und anschliessende Feulgensche Färbung des aspirierten zytologischen Originalmaterials umfasste. Die zytophotometrische Analyse des nuklearen DNS der Tumorzellen im aspirierten Material von Mukoepidermoid-Karzinomen zeigte, dass Tumoren mit höherer Chromosomenzahl, d.h. mit im triploiden Bereich liegenden DNS-Gehalt, eine schlechtere Prognose im Vergleich zu Tumoren mit DNS-Gehalt im diploiden Bereich hatten. Diese Daten werden auch von unseren früheren Befunden gestützt, nämlich dass eine Änderung des DNS-Gehalts der Zellkerne von Tumoren vom dipliden Bereich im triploiden Bereich mit der pro-gnostisch wichtigen Eigenschaft eines invasiven Wachs-tums verbunden ist. Es hat somit den Anschein, als ob die zytophotometrische Analyse des Kern-DNS von aspirierten Tumorzellen wertvolle Aufschlüsse über den Malignitätsgrad des individuellen Tumors geben kann, was dagegen aufgrund der zytomorphologischen Cha-rakteristika allein nicht möglich ist.
