Low muscle mass and BMD are associated with greater risk of fracture, disability and institutionalization. Sarcopenia and osteoporosis are very prevalent in older adults, however, it is not yet known whether osteosarcopenia presents poorer physical function than Sarcopenia only. Aim: We evaluate the physical function of older adults with Osteosarcopenia, osteoporosis only and Sarcopenia only. Method: Cross sectional analyzes of 400 patients from SARCOS study, an observational study of Sarcopenia and Osteoporosis in older outpatients of Federal University of Sao Paulo-Brazil. All patients were underwent DXA analyses. Impaired Physical function was diagnosed by Chair stand test <5-time sit to stand (CHT) or Walking Speed (4.5m) ≤ 20% (LWS), or muscle strength of the dominant ≤ 20% (LMS). Osteoporosis (OP) was diagnosed by WHO criteria. Sarcopenia was diagnosed only by low muscle mass (LMM) according to NIH criteria, Osteosarcopenia by the presence of osteoporosis and LMM. Results: The mean age was 78.22(7.16) yo, and 56.2% women. Osteosarcopenia was almost twice more prevalent in women (61.4%) vs. men (38%) (p<0.001), osteoporosis only was three times higher in women (73%) vs. men (27%) (p<0.001). The Sarcopenia only was similar in both genders. Osteosarcopenic patients were older, slower and weaker compared to Sarcopenia only. Adjuted regression analysis for LWS, Osteosarcopena OR: 3.85(95%IC:0.99–15.01;p=0.052) and sarcopenia OR: 2.22(95%IC:0.84–5.93;p=0.106); for Weakness, Osteosarcopenia OR: 5.96 (95%IC: 2.52 - 14.11; p< 0.001), Sarcopenia OR: 2.53(95%IC:1.16–5.51;p=0.019), for CHT positive, Osteosarcopenia OR: 2.31(95%IC:1.00–5.33;p=0.049), Sarcopenia OR: 1.60(95%IC:0.77–3.32;p=0.202). Conclusion: The Osteosarcopenia has a stronger association with impaired physical function than Sarcopenia only
