摘要
Objective: To assess a specifically design diary self-monitoring (symptoms and peak flow value) mobile app. We evaluate asthma control and early acute exacerbation treatment. We also evaluate app adherence, nonadherence patients profile and control improvements. Material and Methods: We have a free app for both Android and IOS system. Doctor has to subscribe the patient profile. An observational prospective study with no controlled moderate-severe asthma attended previously in a specialized care. Randomized to "control group" (conventional treatment plans with GINA/GEMA guidelines recommendation) and "Case group" (self-care plan on the app). Both groups have clinical visits every 3 months and they do asthma control, quality of life and app satisfaction tests. They can call or go to respiratory day hospital. Results: January 2017, we have 18 patients in "case group" and 21 in "control group" with at least 6 months follow up. It was reported a 76.5% global adherence. 3 patients were lost and another 3 had a less tha 60% adherence. The rest of the sample (n=12) had a 86.5% of mean adherence. Per-group characteristics and previous year comparative (Figure 1). We have reported 340 alarms. 53% from poor adherence group. 55 (16.1%)were answered and 12 day hospital visit was required. The rest were managed by the app. Conclusion: In our sample we have good adherence but improvable. 3 patients with poor adherence but we can intuit they were more severe and had worse psychological profile (Nijmegen). They loss control and generate the most percentage of alarms. There were a global decreased in primary care visit and inpatients. There is an improvement in asthma control (ACT) but they are preliminary results and small sample.
