Objectives: The purpose of this study was to build and implement three-dimensional, total process managed prevention and control system of hypertension, and to improve the effect of hypertension management. Methods: The hypertension prevention and control network has been established since 2013, in which the hospital for prevention and treatment of chronic disease was the pivot to formulate and implement the prevention and control strategies. Hypertension alliance was found among cardiologists and family doctors. The community health service centers as the network terminal integrated the prevention-therapy-management for hypertension patients. Health promotion was improved by series of measures. Moreover, internet-based health management for hypertension was implemented by family doctors groups. Results: The effect of hypertension management was obviously improved after 5-year implementation for the hypertension prevention and control system in Shenzhen Futian, China. The standardized management number of hypertension, the control rate of blood pressure, the drug compliance rate and the self measured blood pressure rate were increased compared with five years ago. By the end of December 2017, 45986 patients with hypertension were managed, the rate of standardized management was 60.52%, the control rate of blood pressure was 65.56%, the drug compliance rate was 53.2%. the self measured blood pressure rate was 65%, the differences were significant (P < 0.01). Conclusion: The three-dimensional, total process managed prevention and control system of hypertension in Shenzhen, China is satisfactory to improved the effect of hypertension management, which is worth extending to other place in China.
